California POLST Form
This is a sample of California’s current Physician Orders for Life-Sustaining Treatment (POLST) Form in English.
*The only change between the 2016 POLST form and the 2017 POLST form is the box of text in the lower right side of the form which reads, “Your POLST may be added to a secure electronic registry to be accessible by health providers as permitted by HIPAA.”
