The POLST Form in Other Formats and Languages

Foreign Language Translations of the POLST Form

While the goal is to eventually have the National POLST form translated into other languages, right now it is just in English. Below are translations from California and Hawaii of their forms, which are similar to the National POLST form. Translated versions are for educational purposes only.

The California POLST Program offers samples of translated POLST Forms:

  • Tagalog

The Hawaii POLST Program offers samples of translated POLST Forms:

  • Chinese (both Traditional Chinese and Simplified Chinese)
  • Ilocano

  • Tongan
  • Vietnamese

Braille POLST Form

  • You can request a braille form from the California POLST Program.
