The Personalize Your Care Act 2.0, introduced June 22, 2016, by Congressman Earl Blumenauer (D-OR) and Congressman Phil Roe (R-TN), proposes to increase accessibility to and public awareness of advance care planning. Personalize Your Care Act 2.0 improves upon the previously introduced Personalize Your Care Act of 2013, including explicit mention of POLST (Physician Orders for Life-Sustaining Treatment) as an element of advance care planning. It also provides a requirement for the inclusion of advance directives and POLST Forms in patient electronic health records to improve accessibility of these advance care planning documents.
